Most people hate giving feedback, me too.
- Giving feedback is difficult
- There are feedback forms with tons of generic questions that you have no idea how to answer
- You _forced_ to do it once per year
Here is the screenshot of sample feedback form. I find it crazy.
But I’ve never seen such simple question in a feedback form – Do I want this guy (or girl) to be your team member?
What if feedback forms look like that?

- What must be improved?
- Do you I want to have him/her as a team member?
